Temporary Food/ Farmers Market Permit
Come in person to the Environmental Health Office to submit your application. You should set aside approximately 30 minutes for this process. Your application should be submitted at at least 14 days before the event.
We are located in the Health & Justice Building at 151 South University Ave, Suite 2600, Provo, Utah 84601. Our office hours are Monday through Friday from 7:30am to 5:30 pm.

Utah County Health Department prefers that applications be processed in the office. If this presents a hardship and you are out of county, the following policy applies:
Applications submitted by fax or mail must be received in our office 14 calendar days before the event. Please note that an additional $10 processing fee will be charged.
Utah County Health Department is under no obligation to accept or process applications submitted by mail or fax less than 14 days prior to the event.
We are currently not accepting any applications by email.
