(Immunizations)

We accept the following health insurances for select vaccines (see your provider for your plan's vaccine coverage):

  • DMBA/ Deseret Secure
  • EMI (Education Mutual)
  • Humana
  • Select Health (IHC plans, Select Med, Select Care, etc.
  • Medicare
  • Medicare Advantage Plans
  • Medicaid (Including PCN)
  • PEHP
  • United Health Care (UHC)

As of January 1, 2012, UCHD can no longer immunize underinsured children (ages 2-18). Vaccinations may be obtained from Mountainlands Community Health Center located in Provo and Payson.
