A 49 year old South Jordan woman died when a load of cement forms fell on her while they were being unloaded.
On November 24, 2014, at 1104, Deputies with the Utah County Sheriff's Office were dispatched to a report of a woman who was injured when a stack of cement forms fell on her. The forms were on a flat bed, full size trailer from a tractor-trailer rig when they were being unloaded. The woman was standing on one side while the forms were being unloaded from the opposite side. As the stack was lifted it shifted and about 30 forms fell on her. The forms are about 2' x 8' and weigh about 70 pounds each. Witnesses immediately called 911 and began to remove the forms to free the woman. She suffered extensive injuries and medical personnel from Unified Fire Authority began treating her. LifeFlight responded but after attempting to revive the victim she was pronounced dead at the scene. The victim is Cynthia "Cindy" Weeks, age 49, of South Jordan. Her body was taken to the Medical Examiner's Office in Salt Lake City. There is nothing suspicious about the circumstances.
